- arrange transportation for medical or dental appointments
- understand your diagnosis and your doctor’s plan for your health care
- understand how your medications work and possible side effects; compose a comprehensive medications list
- set up your medications in a medication box
- prepare for your doctor’s appointment or come along with you as an extra set of ears
- provide support and education for healthy lifestyle changes
- locate community resources when you need extra help
- check your blood pressure and provide basic nursing assessment when you have concerns
