Lifesaving Information For Emergencies
Vial of Life is a program that aims to make vital health information available to emergency responders in the event of a medical emergency. The Vial of Life is designed to speak for you when you can’t speak for yourself. The vial contains important medical information that can assist emergency personnel in administering the proper medical treatment.
The vial is a basic medical history sheet that is filled out and kept in a prescription vial (in your refrigerator door). The red Vial of Life decal is placed on the outside of the refrigerator in the top corner and/or on your entrance door.
The first thing first responders want to know is your list of medications. The difference between life and death when you’ve had a heart attack could be a copy of your previous EKG. Even during normal doctor visits, the Vial of Life will help you tell the doctors and nurses what medications you are taking, and the ailments that you have.
The Vial of Life details your relevant medical information, including:

How it works:
1. Get your Vial of Life kit.
Call or stop by Trego Hospital Endowment Foundation, Inc.
2. Fill out the Vial of Life medical information form.
You may want to ask your doctor to help you. Check the form for accuracy. Take it to your pharmacist to check for current information and compliance. Make sure to update your form if your health changes so that it is always accurate and up-to-date. If you already have a Vial of Life kit and need a new form, you can download one below.
3. Place your medical information form in the Vial of Life container, or in a plastic baggie.
You may want to also include your photo I.D. and other important medical documents. You can also include POA, DNR, and Living Will.
4. Place your Vial of Life container in your refrigerator.
Emergency personnel and first responders are trained to look on the first shelf of door your refrigerator so it is the very best place for this vital information. Everyone has a refrigerator, and we can all expect to find it in the same place, making it easy to find–and even after a disaster, it will usually be intact.
5. Place a decal or magnet on your refrigerator door.
The UPPER RIGHT FRONT is recommended for decal placement.
6. Place a decal on your front window or door.
The decal will let emergency medical responders know that you have a Vial of Life.
7. Place your house key on your Vial of Life keychain.
If you are out running errands, at a restaurant, etc. and an emergency arises, emergency medical responders will be alerted that you have Vial of Life. You may also want to keep a medical information card in your purse or wallet.
When you can’t speak for yourself, when you are confused, in pain, or scared, the Vial of Life can speak for you.
